Modern Day Wellness is a medical practice located in Suite 124 of the DEXAFIT building at 5656 S. Power Rd. in Gilbert. The suite sits in the north building of the complex, off E Galveston St, near East Valley ER. The practice shares a parking lot with ROC Physical Therapy and Desert Springs Dental, and parking is on site. Office hours run Monday through Friday, 9:00am to 5:00pm, and later appointments can be requested. The practice describes itself as small by design, so patients see the same clinician at each visit.

The practice organizes its care into several areas. For women, hormone services include bioidentical replacement, testing for hormonal imbalance, management of estradiol and progesterone, testosterone therapy for women, hormone pellet insertion, and screening for thyroid problems including Hashimoto's. Men's care focuses on evaluating testosterone and replacing it with ongoing monitoring and dose changes, alongside thyroid and metabolic screening, vitamin D and B12 testing, and peptide therapy. The medical weight loss program combines metabolic and hormone lab work with supervised treatment, which may involve GLP-1 and GIP medication, and it also takes on patients transferring existing GLP-1 care. Oral appetite suppressants are prescribed only after an evaluation and with follow-up visits on a set schedule.

Aesthetic treatments include neuromodulators such as Botox and Xeomin, given in low doses to soften frown lines, forehead lines and lines around the eyes. Hyaluronic acid fillers, including Juvéderm, are placed to rebuild lost volume. Other options are chemical peels and microneedling combined with PRP or PRF. The office dispenses medical-grade skincare from three brands, SkinMedica, PCA Skin and EltaMD, with a product match made to each patient's skin during consultation. Also available are IV hydration, vitamin replacement, and correction of B12 and vitamin D levels. On-site lab work covers full hormone panels, metabolic and insulin markers, vitamin D and B12, iron, a complete blood count, and a thyroid panel.

A typical course starts with labs drawn before the consultation, so the visit can review results and the options that follow from them. Treatment plans begin conservatively and are adjusted using repeat labs. Office visits are cash-pay and are not billed to insurance. Bloodwork is billed through insurance or the practice's negotiated rates, and both prices are shown before any draw. Medications are sent to the patient's pharmacy and billed there, and insurance can be used for them. Payment is due after the visit by credit, debit, HSA or FSA card, and a prepay link is available on request. Published prices may change, and any added injections or extra panels are priced before they happen.

Telehealth follow-up visits are available to patients located in Arizona, and some lab draws and visits take place in Gilbert. Transfers from another prescriber are welcome; records and current labs are reviewed before care continues. Testosterone is a Schedule III controlled substance and is prescribed only after evaluation and with ongoing monitoring. Prescriptions are not offered or implied online, and peptides cannot be ordered online either.
