Amy Berkhout

Amy Berkhout is the owner and a Family Nurse Practitioner at Divine IV and Wellness, which she founded. She lists DNP, FNP-BC and PMGT credentials. Her training began with a bachelor's degree in health promotion at Northern Arizona University, followed by a Bachelor of Science in Nursing in 2004 and a Master of Science in Nursing as a Family Nurse Practitioner from the same university in 2013. She received her doctorate there in spring 2025. She specializes in IV therapy, hormone replacement therapy, weight loss and thyroid disorders. She has specialty training in nutritional IV therapies, hormone replacement and thyroid optimization, and in joint injections, trigger point injections and platelet-rich plasma therapy. She co-owns and operates East Valley Joint Clinic, founded in 2016. At Divine IV and Wellness she performs laser tattoo removal with the Harmony XL PRO system, consults on TED hair restoration, recommends regular medical-grade facials, and evaluates patients' health and weight loss goals for semaglutide injections. Owning her own practice lets her spend more time with each patient. Her methods are holistic and preventive, rooted in health promotion and disease prevention.
