Booking is self-pay only, as the practice does not accept insurance. Cash, check and major credit cards are accepted, with payment due at the time of service. A 10% courtesy discount applies to upfront, fully self-paid procedures. Financing is available through CareCredit, PatientFi and Cherry Financing, as well as in-house monthly installments. Intake forms, including registration, medical history and HIPAA consent, are completed through the online portal before the visit. Clients are asked to bring a valid state-issued driver's license, military ID or passport and to arrive 15 minutes early. Cancellations and reschedules need 24 hours' notice, or a $50 short-notice fee applies, with exceptions for sudden medical emergencies. Arriving 15 or more minutes late may mean rescheduling or a shorter appointment.
