About
Candace Herrin founded Hometown Primary Care in November 2022 and leads the Choctaw clinic as a family nurse practitioner. She is board-certified as a family nurse practitioner, holds the credentials APRN and FNP-C, and is licensed in Oklahoma and Florida.
With 16 years in healthcare, she spent 10 years as a registered nurse, including 10 years in emergency medicine as an ED RN and charge nurse, and has practiced for 6 years as a family nurse practitioner. Her background also includes pulmonology, sleep medicine, OB/GYN, and urgent care before she opened her own practice.
The clinic grew out of her concern about rushed appointments and fragmented care elsewhere. Her approach centers on longer visits and getting to know each patient over time.
