Revive MD began in men's sexual health and later grew into hormone optimization and overall wellness, including care for patients' partners and family members. The aesthetics menu includes Exion body and face treatments, RF microneedling, Clear RF and Emfemme, along with HydraFacial, Botox, chemical peels and hyaluronic acid filler. These non-invasive skin rejuvenation and anti-aging services are led by Swetha Nataraj, MD. The $100 initial aesthetics consultation is credited toward any aesthetic procedure booked afterward. Aesthetic services are private pay only.

Visits are requested through online booking or secure message. Men and women can complete an online symptom assessment before scheduling. A $50 booking hold is collected at scheduling and applied toward the consultation fee. The office is open Monday through Friday, 9:00 am to 4:00 pm. Office consultations and follow-up visits are billed through insurance, and many major plans are accepted. Labs and prescriptions are billed to insurance as usual. Insurance patients are offered the next available appointment once eligibility is verified, while private pay patients may be scheduled within 7 business days. Flexible spending and health savings account cards are accepted. Once private pay is chosen for a visit, it cannot be switched to insurance later.
