The practice was started by two emergency physicians, Dr. Greg Moreno and Dr. Keith Yablonicky, after they saw patients slip through gaps in conventional medicine.

Urgent care is open daily from 8:00 AM to 8:00 PM at San Clemente and Carlsbad, and walk-ins are welcome for everyday illnesses, minor injuries and unexpected concerns. Services include X-rays, sutures and wound repair, fracture reductions, splinting and casting, IV antibiotics, sports physicals with concussion evaluation, immunizations and pediatric urgent care. Each location has in-house digital X-ray with results shared during the same visit, and an on-site CLIA-accredited lab where many results are ready the same day. Reserved appointment slots come with a membership, and their booking fee is separate from copays, visit fees and insurance.

Longevity medicine starts with an initial assessment completed in a single day, with more than 30 advanced diagnostics. Testing covers advanced blood and hormone panels, inflammation markers, genetic testing, DEXA body composition, VO2 max testing, glucose monitoring, EKG, CT cardiac angiogram review and movement assessment. Results are reviewed one-on-one with a physician to build a personalized profile and custom plan. Membership follows a pre-visit consultation, a day-one assessment, a physician results review about two weeks later and a month 6 check-in. Physician-supervised peptide therapy is offered within these programs.

Aesthetic wellness covers neurotoxins and fillers, RF microneedling and radiofrequency skin tightening, laser hair removal, laser skin treatments, IPL for pigmentation and veins, facials and chemical peels, PRP-based hair loss therapy, SculpSure noninvasive body contouring and IV hydration, with NAD+ therapy and medical-grade skincare also available. Wellness and medspa visits are booked ahead. San Clemente longevity and aesthetic services run Monday through Thursday, 8:00 AM to 5:00 PM. Carlsbad keeps Monday through Thursday hours from 10:00 AM to 6:00 PM and Friday from 10:00 AM to 4:00 PM. Both are closed on weekends.

Signature membership is $350 per month with a 6-month minimum and includes 20+ advanced health assessments, an annual longevity consultation, a coronary screening review, 2 DEXA scans, 1 VO2 max test and 4 vitamin IVs per year. Executive membership is $720 per month and adds 30+ assessments, the Galleri multi-cancer detection test, 4 DEXA scans, 2 VO2 max tests, 12 vitamin IVs and a yearly 3-month continuous glucose monitor protocol. Both require a $750 deposit to begin the assessment and include unlimited concierge primary care, urgent care and telemedicine 7 days a week from 8:00 AM to 8:00 PM, unlimited focused red light therapy, and member pricing on hormone replacement therapy, weight loss, medspa services and supplements. Urgent care procedures and tests are billed separately. The urgent care membership is $25 per month for a primary adult and $10 per month for children.
