The Jacksonville office is open Monday through Friday, 8:00 AM to 5:00 PM, at 904-355-1553. The Fernandina Beach office is open Monday through Friday, 9:00 AM to 5:00 PM, at 904-277-8370. New patients are asked to bring photo ID, insurance cards, a list of medical conditions and medications, and any copay, co-insurance or deductible payment. Established patients bring the same items to each appointment. New patient forms are completed through the patient portal, and login is arranged by calling the office. The practice accepts Blue Cross Blue Shield, Medicare and most insurance plans, and lists others including Aetna, AARP, Cigna, Humana and Wellcare.
