CAN Community Health Inc. describes itself as a health organization focused on HIV, STIs and hepatitis C. It says its mission is to expand access to comprehensive health services through prevention, treatment and support, and its stated vision is to improve health outcomes through prevention and comprehensive care. The organization lists five guiding principles: communicate, advocate, research and innovate, execute, and sustain.
The organization describes its model as integrated, with care built around each patient's needs and a view of the whole person. Medical care is provided by attending physicians working alongside nurses and other specialized providers, and covers acute and long-term HIV management, primary care for people living with HIV, psychiatric services, clinical trials and referrals to specialists.
Services span several areas. Dental care is offered at selected locations and includes exams, cleanings, digital X-rays, fillings, crowns, bridges, root canal therapy, periodontal disease care, extractions and wisdom teeth removal, dentures, and oral cancer screening. Behavioral health offers counseling and therapy for individuals, couples and groups, as well as medication management and talk therapy, with telehealth available for behavioral health. The in-house pharmacies let patients pick up prescriptions at their appointments.
Testing is a central part of the work. The organization offers free, confidential HIV, STI and hepatitis C screening, which can be booked online by choosing a state and nearby location, and provides rapid HIV and hepatitis C testing in community settings. Current patients receiving PrEP, HIV care or hepatitis C treatment are asked to call their clinic directly instead of using the testing booking page.
Case management helps patients find out whether they qualify for state and federal programs, build care plans, arrange referrals, and get help with insurance and housing. Education and community programs include HIV education for schools and community groups, a free Persons Living With HIV/AIDS Speakers' Bureau, and VOICES/VOCES, a single-session group video session on condom use and safer sex.
Describes an integrated, patient-centered model that treats the whole patient and sets services according to each person's needs, with a mission centered on prevention, treatment and support.
